Customizing your Forms

Many of you would like to customize your forms in several ways.  The CoursEval system allows you to set your own open and close times for the survey, and to add extra questions to your course evaluations forms, particularly to receive more formative feedback on things such as courses assignments or course materials.  This is reasonably easy to do!  Here are some steps.

1.  When you log into the site, if you have already indicated to us what form you want to use, you will see a question mark in upper right hand corner of the screen:

When you click on the question mark, you will see the available surveys.  If the upcoming survey is not there, it means that we have not yet received the word to link your course to the survey.  You will see places to set custom times, and to add questions:

2.  To set custom times, select the “set open/close” icon. If you select a different opening time, you will need to let your students know that the survey is available.  Simply send them an email with this link, urging them to complete the survey:  https://courseeval.wooster.edu/etw/ets/et.asp?nxappid=WCQ&nxmid=start

3.  To add questions, Look at the questions included in the survey form form you chose.  These are the “core questions.”  

4.  Scroll down further in the screen and look at the available extra questions.  You may check up to ten of these to add.  If you click on the highlighted word to the right, a pop-up screen will show you how the question is worded.  If it says “comment”, that means students will simply get a text box to fill in:

5.  If there is a question that you would like that is not on the list, you can request it by simply typing the question into the field at the bottom of the page.  We ask that additional questions be generic and fairly simple, so that they can be available to others:
